ADA Accommodation
Please complete the attached ADA Accommodation Request Form (COQB). This document is divided into two sections. The top section should be completed by the examinee, and the second section must be completed by the qualified provider or professional with knowledge of the accommodation.

ESL Accommodation
Those who are approved for ESL Accommodation by OSFM Staff will receive time and one-half (1.5X) for their exam. For example, if an exam time limit is 3.5 hours, the ESL Accommodation will allow the test taker to have 5.25 (5 hours and 15 minutes). Please read and follow the instructions in the attached document to submit a request for an ESL Accommodation.
